Position Summary Maria Joseph Nursing and Rehabilitation is seeking an organized and compassionate Discharge Planner to coordinate safe, timely, and effective discharge plans for our residents. This role serves as a key liaison between residents, families, healthcare providers, and community resources to ensure continuity of care after discharge. Key Responsibilities Coordinate and manage resident discharge planning in collaboration with the interdisciplinary care team. Assess resident needs and arrange appropriate post-discharge services such as home health, DME, therapy, hospice, or follow-up appointments. Communicate discharge plans clearly with residents and families. Ensure all discharge documentation is complete, accurate, and compliant with regulations. Work closely with physicians, nursing staff, therapy teams, and insurance providers. Monitor discharge outcomes and address barriers to safe transitions of care. Maintain compliance with state, federal, and facility regulations. Qualifications Experience in skilled nursing, case management, discharge planning, or healthcare coordination preferred. Strong organizational, communication, and documentation skills. Knowledge of community resources and post-acute care services. Ability to manage multiple cases while maintaining attention to detail. Compassionate, resident-focused approach to care transitions.
