The QIDP job duties include, but aren't limited to: creating and revising resident's individual service plans, communicating with guardians, families, and all members of the team, coordinating care with the team, ensuring your residents have what they want/need, helping assist with care/dining as needed, advocate for your residents, and ensuring our residents have the most enriched lives that they can.

Qualifications:
