Minimum 2 Year Required About the Role We are seeking an experienced Registered Nurse (RN) Case Manager for a 13-week contract position in Sitka, Alaska. This ambulatory nursing role supports Specialty Surgery services and focuses on patient assessment, care planning, education, care coordination, medication administration, and continuity of care. The RN Case Manager will collaborate with providers and interdisciplinary teams to coordinate referrals and follow-up care, support patients navigating the healthcare system, and maintain accurate clinical documentation throughout the continuum of care. Responsibilities Perform comprehensive nursing assessments and develop, implement, evaluate, and revise individualized plans of care. Coordinate referrals, follow-up appointments, specialty services, and transitions of care in collaboration with providers and interdisciplinary teams. Administer medications and treatments, monitor patient response, and recognize changes requiring intervention or escalation of care. Educate patients and families regarding medications, procedures, treatment plans, discharge needs, and ongoing care management. Maintain timely and accurate documentation using Meditech while adhering to patient safety, infection prevention, confidentiality, and regulatory standards. Requirements Active Alaska Registered Nurse (RN) License Required at Time of Submission. Graduate of an Accredited School of Nursing Required; Bachelor of Science in Nursing (BSN) Preferred. Minimum 1 Year of Registered Nursing Experience Required. Current
BLS/BCLS
Certification Required; Case Management Certification Preferred. Candidate Must Reside at Least 100 Miles From the Facility and Be Able to Secure Their Own Housing and Transportation. The RN Case Manager will provide coordinated, patient-centered ambulatory nursing care while supporting continuity, safety, and positive outcomes for specialty surgery patients.
