A Registered Nurse provides medical care and treatment, educates patients about their conditions, and provides emotional support to patients and families. Works in hospitals, schools, clinics or community health centers. Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.
Nurse Practitioner - Capstone Health Capstone Health seeks a Certified Family Nurse Practitioner to join the Capstone team. The ideal candidate is service orientated and patient driven, enjoys working with underinsured patients and is culturally sensitive. This position is designated as weekends only (8am-5pm, Saturday and Sunday).
POSITION SUMMARY
The Nurse Practitioner (NP) provides comprehensive primary and preventive healthcare services to patients within the scope of licensure, certification, and collaborative practice agreements. The NP performs health assessments, diagnoses and treats acute and chronic medical conditions, orders and interprets diagnostic studies, prescribes medications, provides patient education, and coordinates care with physicians and other members of the interdisciplinary healthcare team. The Nurse Practitioner supports integrated, patient-centered care by promoting quality improvement, patient safety, risk management, and regulatory compliance consistent with Capstone Health's mission, vision, values, HRSA Health Center Program requirements, and organizational standards.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Clinical Services Perform comprehensive health assessments, including obtaining medical histories, conducting physical examinations, evaluating, diagnosing, and treating acute and chronic medical conditions within the scope of practice. Develop, implement, evaluate, and revise individualized treatment plans based on patient needs and evidence-based clinical practice. Order, perform, and interpret laboratory studies, diagnostic imaging, and other diagnostic procedures as appropriate. Prescribe medications, therapeutic interventions, and medical treatments in accordance with state law, collaborative practice agreements, and organizational policies. Perform approved clinical procedures within the scope of licensure and competency. Provide preventive healthcare services, chronic disease management, health maintenance, immunizations, and patient education. Conduct follow-up visits to monitor patient progress, treatment effectiveness, and medication management. Provide telehealth services in accordance with organizational policies and applicable regulatory requirements. Provide emergency assessment, stabilization, and appropriate intervention within the scope of practice until a higher level of care is available. Participate in reciprocal after-hours call coverage for Capstone Health and Whatley Health Services patients in accordance with organizational scheduling requirements. Care Coordination Collaborate with physicians through established collaborative practice agreements to provide comprehensive patient care. Coordinate referrals to specialists, hospitals, diagnostic services, behavioral health providers, and community resources as appropriate. Collaborate with nurses, care coordinators, behavioral health providers, pharmacists, and other interdisciplinary team members to promote continuity of care. Participate in interdisciplinary case reviews, care planning, and treatment coordination. Educate patients and families regarding diagnoses, treatment plans, medications, preventive care, and healthy lifestyle practices. Clinical Leadership Provide clinical consultation and guidance to nursing and clinical support staff. Assist with orientation, mentoring, training, and competency development of clinical staff. Support approved student clinical rotations and educational activities, when applicable. Participate in community health education, prevention initiatives, and wellness programs. Documentation and Reporting Maintain accurate, timely, and complete clinical documentation within the Electronic Medical Record (EMR). Document patient assessments, diagnoses, treatment plans, procedures, medications, patient education, and follow-up care in accordance with organizational policies and regulatory requirements. Maintain records in compliance with HIPAA, payer requirements, and applicable federal and state laws. Assist with quality reporting, clinical performance measures, and organizational reporting requirements. Quality, Risk Management, and Patient Safety Participate in organizational quality improvement, performance improvement, compliance, patient safety, and risk management initiatives. Promote evidence-based clinical practice and continuous quality improvement. Identify and communicate patient safety concerns, compliance issues, operational risks, and barriers to care through established reporting processes. Participate in audits, accreditation reviews, peer review, corrective action initiatives, and performance improvement activities. Support compliance with HRSA Health Center Program requirements, FTCA risk management expectations, HIPAA, OSHA, state licensing requirements, and organizational policies. Provide quality and patient safety oversight within assigned clinical responsibilities. Collaboration and Professionalism Demonstrate professionalism, integrity, ethical clinical practice, and respect for patients, families, and colleagues. Promote the mission, vision, and values of Capstone Health. Establish and maintain collaborative working relationships with patients, families, healthcare providers, community partners, and staff. MINIMUM QUALIFICATIONS
Education Required Master's or Doctoral degree in Nursing from an accredited Nurse Practitioner program. Licensure and Certifications Required Current unrestricted Registered Nurse (RN) license in the State of Alabama. Current Alabama Advanced Practice Registered Nurse (APRN) license. National certification appropriate to the assigned specialty (e.g., Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), Psychiatric Mental Health Nurse Practitioner (PMHNP)). Current DEA registration, if prescribing controlled substances. Current Alabama Qualified Alabama Controlled Substances Certificate (QACSC), if applicable. Current Basic Life Support (BLS) certification. Preferred Advanced Cardiac Life Support (ACLS), when appropriate to the practice setting. Experience Required Minimum of two (2) years of clinical Nurse Practitioner experience or equivalent clinical experience appropriate to the assigned specialty. Preferred Experience in a Federally Qualified Health Center (FQHC), community health center, or primary care setting. Experience with Electronic Medical Record (EMR) systems. Experience providing telehealth services. Experience working within an interdisciplinary healthcare team. Knowledge, Skills, and Abilities Knowledge of primary care, preventive medicine, chronic disease management, and evidence-based clinical practice. Knowledge of applicable federal and state laws, regulations, and professional standards, including
HIPAA, OSHA, HRSA
Health Center Program requirements, collaborative practice requirements, and organizational policies. Ability to perform comprehensive health assessments, diagnose medical conditions, develop treatment plans, and provide patient-centered care. Strong clinical judgment, critical thinking, diagnostic, and problem-solving skills. Excellent verbal, written, and interpersonal communication skills with the ability to establish effective relationships with patients, families, and healthcare team members. Ability to effectively manage urgent and emergent clinical situations within the scope of practice. Strong organizational and time management skills with the ability to prioritize multiple clinical responsibilities. Proficiency with Electronic Medical Record (EMR) systems, clinical documentation, and standard computer applications. Ability to collaborate effectively with physicians, nurses, behavioral health providers, pharmacists, care coordinators, and other interdisciplinary team members. Commitment to continuous learning, quality improvement, patient safety, and professional development.
WORK ENVIRONMENT
Work is performed primarily in outpatient clinic settings. Frequent interaction with patients, families, healthcare providers, and community partners. Regular use of Electronic Medical Records, computers, telephones, and clinical equipment. Occasional evening, weekend, or after-hours work may be required to support patient care and call coverage.
PHYSICAL REQUIREMENTS
Ability to remain stationary for extended periods while evaluating patients and completing documentation. Ability to move throughout clinic locations to provide patient care. Ability to communicate effectively in person, electronically, and by telephone. Ability to operate standard office equipment, clinical equipment, and computer systems. Occasionally lift or carry medical supplies or equipment weighing up to 20 pounds. Ability to perform the essential functions of the position with or without reasonable accommodation.
