This full-time, 13-week contract position offers a balanced Monday through Friday schedule from 8:00 AM to 5:00 PM. Field-based, youll travel daily to patient homes within your assigned territory, supported by mileage reimbursement. Nurses typically manage a caseload of 30 points each week, translating to about 35 patient visits per day.

Key Responsibilities:

Complete comprehensive OASIS assessments and develop tailored care plans. Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management. Monitor patient status, identify and report changes promptly to care teams and physicians. Provide education to patients and families regarding disease management, care plans, medications, and preventive health strategies. Accurately document patient care and assessments using Home Care Home Base (HCHB). Collaborate closely with interdisciplinary teamsphysical therapists, occupational therapists, social workers, and home health aidesfor coordinated patient support. Attend case conferences, staff meetings, and continuing education to stay current in care practices. Uphold all clinical, regulatory, and agency standards for patient services and documentation.

Requirements:

Active Registered Nurse (RN) license in good standing for the state. At least one year of recent home health field experience. Proficiency with Home Care Home Base (HCHB) and OASIS required. Dependable transportation, valid drivers license, and current auto insurance.
