Job Description

Mercy is seeking an RN Care Manager to coordinate and optimize patient care across the continuum in our Healthcare & Medical Services department. In this role, you will perform comprehensive assessments, develop and implement care plans, facilitate transitions of care, and collaborate closely with physicians, interdisciplinary teams, patients, and families. You will monitor outcomes, manage utilization, support patient education, and ensure adherence to evidence-based guidelines while advocating for patient needs. Responsibilities Perform comprehensive nursing and psychosocial assessments for assigned patients Develop, implement, and update individualized care plans Coordinate care and services across inpatient, outpatient, and community settings Facilitate safe and timely transitions of care and discharge planning Collaborate with physicians and interdisciplinary teams to optimize outcomes Monitor utilization and support evidence-based, cost-effective care Educate patients and families on conditions, treatments, and self-management Advocate for patient needs and remove barriers to care Document all care management activities in the EMR accurately and timely Track outcomes and participate in quality and performance improvement initiatives Required Skills Care coordination Patient assessment Discharge planning Utilization review Chronic disease management Electronic Medical Records (EMR) documentation Interdisciplinary team collaboration Patient and family education Data collection and outcome monitoring Knowledge of healthcare regulations and payer requirements
