Schedule/Hours:

Full-time, Monday through Friday On-call rotation may be required

Key Responsibilities:

Deliver in-home nursing care, skilled interventions, and personalized education Complete patient assessments and thorough OASIS documentation accurately and on schedule Collaborate with physicians and multidisciplinary teams to implement tailored care plans Manage wound care, medication administration, disease monitoring, and health education Communicate pertinent changes in patient condition to the clinical team promptly Maintain organized, timely, and compliant documentation for each visit

Qualifications:

Current, active RN license At least 1 year of recent home health RN experience Demonstrated expertise with OASIS documentation Experience with HomeCare HomeBase strongly preferred Ability to independently manage a structured daily caseload Reliable transportation and valid drivers license required
