Job Description

Deliver compassionate care in a dynamic home health environment, utilizing your nursing expertise to help patients achieve their highest quality of life. This 13-week, full-time contract offers an engaging role supporting individuals in their homes while collaborating within a multidisciplinary healthcare team.
Schedule:
Monday through Friday, 8:00 AM 5:00 PM Daily travel between patient homes (mileage reimbursement available) Manage a caseload of 30 points per week (typically 35 patient visits per day)
Qualifications:
Current, active RN license (state of practice) in good standing At least 1 year of recent Home Health nursing experience (field work preferred) Solid experience and proficiency with Home Care Home Base (HCHB) electronic documentation OASIS assessment experience required Reliable vehicle, valid drivers license, and proof of auto insurance
Key Responsibilities:
Complete thorough OASIS assessments and develop personalized care plans for each patient Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management Monitor patient progress, promptly communicate significant observations to the care team and physicians Educate patients and their families on conditions, medications, and care instructions Maintain accurate, detailed documentation using Home Care Home Base (HCHB) Foster a team approach by working closely with physical therapists, occupational therapists, social workers, and home health aides Participate in interdisciplinary meetings, case conferences, and ongoing education Ensure compliance with agency and regulatory requirements for care and documentation

Your expertise will help patients thrive safely at home. $2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments. Eligible candidates may also receive healthcare benefits, housing, and meals/incidentals.
