  • Monday-Friday, 8:00 AM
  • 4:30 PM +
  • Hours:
  • 40 hours per week guaranteed +
  • Pay Rate:
  • $53.00
  • $55.90 per hour, plus mileage reimbursement between patient visits +
  • On-Call Requirement:
  • Yes, 4:30 PM
  • 8:00 AM Monday-Friday rotation every other week +
  • Weekend Requirement:
  • Possible weekend rotation
  • Responsibilities
  • + Manage a hospice caseload of up to 15 patients + Perform routine patient visits and comprehensive nursing assessments + Complete hospice admissions and start-of-care visits + Provide end-of-life care and support to patients and families + Perform death visits and post-death care as needed + Develop and update individualized care plans in collaboration with the interdisciplinary team + Coordinate care with physicians, caregivers, social workers, and other healthcare professionals + Maintain timely and accurate documentation in Homecare Homebase (HCHB) + Participate in on-call and weekend rotations as assigned •Requirements•+ Active Arizona RN License or Compact RN License + Recent Hospice nursing experience + Recent Homecare Homebase (HCHB) experience + Current CPR/BLS certification + Valid driver's license, vehicle insurance, and reliable transportation + Strong clinical assessment and documentation skills + Ability to work independently in the field •Preferred Skills•+ Experience managing a hospice caseload + Experience performing admissions and death visits + Strong care coordination and case management skills + Excellent communication and patient education abilities + Ability to thrive in a fast-paced home-based care environment •Why Work for Adecco?
