In this vital role, you will utilize your extensive nursing expertise to assess patient needs, administer treatments, and coordinate care plans that promote health and recovery. Duties Conduct comprehensive patient assessments including vital signs, physical examinations, and health history documentation using advanced EMR (Electronic Medical Record) systems. Administer medications via injection, IV infusion, catheterization, and other methods following aseptic technique and HIPAA regulations to ensure patient safety. Perform specialized procedures including suturing, tube feeding management, ostomy care, and wound dressing changes in accordance with best practices. Assist in diagnostic evaluations by collecting specimens for laboratory processing and supporting imaging procedures like sonography or mammography when necessary. Manage patient care plans for diverse populations including geriatrics, pediatrics, behavioral health patients, and individuals with developmental disabilities or dementia. Provide education on health coaching, discharge planning, medication administration, and chronic disease management tailored to each patient's needs. Experience Proven nursing experience in a home health setting (willing to train). Skilled in working with EMR/EHR systems. Experience in specialized fields including hospice & palliative medicine is highly valued. Demonstrated ability to perform advanced procedures such as ventilator management, gastrostomy tube feeding, or sterile processing techniques. Background working with diverse populations across settings like assisted living facilities, nursing homes, home & community care environments, or teaching hospitals. Strong physiology knowledge combined with excellent patient assessment skills; capable of triage during urgent situations while maintaining calm under pressure.
