Fresno and surrounding areas Job Summary The Registered Nurse (RN) is responsible for providing skilled nursing care and case management services to patients in their homes in accordance with physician orders and the plan of care. The RN collaborates with the interdisciplinary team, patient, and caregivers to evaluate, implement, and monitor individualized patient care plans while ensuring compliance with Medicare Conditions of Participation and California state laws. Essential Duties and Responsibilities Conduct initial and ongoing comprehensive assessments of patients in their homes as well as follow-up visits as needed Develop, implement, and evaluate individualized plans of care in collaboration with the Physician and interdisciplinary team. Monitor patient symptoms and note changes in condition, including appetite, mobility, mood, and cognition. Provide competent nursing care and interventions according to industry and state standards. Supervise care provided by Home Health Aides (HHAs) and Licensed Practical Nurses (LVNs), including required supervisory visits. Provide skilled nursing services in the home including, but not limited to wound care, labs, medication management, administration and education, disease and symptom management, infusion therapy (as applicable), catheter care. Document patient progress and update care plans accordingly. Participate in quality assurance and performance improvement activities. Communicate with physicians and the interdisciplinary healthcare team. Attend mandatory staff meetings, case conferences, and in-service trainings. Offer emotional support and guidance to patients and families during difficult times. Documentation and Compliance Update to 24 hours per agency policy-Submit all visit documentation and weekly invoices by Friday of the current week. Maintain compliance with Medicare, ACHC, HIPAA, and California Board of Registered Nursing requirements. Adhere to mandatory reporting laws, including suspected abuse or neglect. Ensure clinical practices align with California's RN scope of practice. Use electronic health records (EHR) or software systems for accurate data entry and tracking. Adhere to compliance with all agency policies. Mandatory Reporting As a mandated reporter under California law, the RN is required to report any suspected abuse, neglect, or exploitation of patients, including elder or dependent adult abuse, to the appropriate authorities. Training and guidelines on reporting requirements will be provided during orientation. Work Environment This is a field-based position with regular travel to patient homes. RNs may be exposed to various home environments throughout their assigned geographic areas. Driving Requirements This position requires regular travel to patient homes. Employees must have a valid driver's license, reliable transportation, and current auto insurance that meets California's minimum liability coverage requirements. Proof of insurance must be provided upon hire and may be requested at any time during employment. Weekend and Holiday Availability Due to patient care and operational needs, employees are required to work a minimum of one major holiday per year and participate in a rotating weekend schedule to ensure consistent and equitable coverage. Qualifications Valid and active California Registered Nurse (RN) license. Minimum of one (1) year clinical nursing experience, preferably in home health. CPR certification (American Heart Association required). Reliable transportation and valid California driver's license.

Pay:

$50.00 - $55.00 per hour Expected hours: No less than 30.0 per week
