In this vital role, you will assess patient needs, develop personalized care plans, and deliver skilled nursing interventions to promote health, recovery, and independence. Your energetic approach and dedication will foster positive patient outcomes while supporting families and caregivers through education and guidance. This position offers an exciting opportunity to work autonomously within a collaborative team dedicated to excellence in home-based healthcare. Duties Conduct thorough patient assessments including vital signs, physical examinations, and health histories to determine individual care needs. Develop and implement personalized care plans aligned with physicians' orders and patient goals, ensuring compliance with all regulatory standards. Administer medications via IV infusion, injections, or other routes while adhering to aseptic techniques and safety protocols. Provide specialized treatments such as wound suturing, ostomy care, catheterization, dialysis management, and ventilator support as required. Educate patients and families on disease management, medication administration, nutrition, mobility techniques, and safety measures to promote self-care. Monitor patient progress through ongoing evaluations; adjust care plans accordingly to optimize recovery or manage chronic conditions. Document all patient interactions accurately within Electronic Health Records (EHR) systems like Epic or Cerner while maintaining HIPAA compliance. Qualifications Valid RN license in good standing with current state certification. Proven experience in home health nursing with familiarity in medical documentation and EHR/EHR systems such as Epic or Cerner. Critical care experience including ICU or Level I/II Trauma Center exposure highly preferred. Knowledge of diverse clinical areas including cardiology, ophthalmology, infectious disease care, geriatrics, behavioral health, and palliative medicine. Skills in IV insertion, medication administration (including botulinum toxin treatment), wound suturing, tube feeding management, and aseptic technique. Experience working with individuals with disabilities or developmental delays; familiarity with long-term care settings like nursing homes or assisted living facilities is advantageous. Strong assessment skills with the ability to triage effectively in urgent situations; proficiency in ICD coding (ICD-9/10) is a plus. Excellent communication skills combined with a compassionate approach to patient-centered care across diverse populations including pediatrics and geriatrics.
