Key Qualifications:
Current, active RN license in good standing (state-specific or compact). Experience in case management with strong clinical assessment, care coordination, and discharge planning skills. Proficiency in interdisciplinary teamwork, patient/family education, and care transition planning. Excellent communication, documentation, and critical thinking abilities. Previous travel RN experience is a plus but not required.
Primary Responsibilities:
Assess patient needs and coordinate individualized care plans from admission through post-discharge. Collaborate with medical teams, social workers, and insurance providers to optimize patient outcomes and resource utilization. Facilitate communication between patients, families, and care providers. Ensure compliance with regulatory standards and organizational protocols. Contribute to process improvement initiatives and maintain thorough documentation.
