Dublin, CA Job Details Part-time From $50 an hour 1 day ago Qualifications Pain medicine Electronic health records (EHR) management RN License Quality improvement Aseptic technique Epic Full Job Description At Your Service Home Health is looking for a compassionate Home Health Registered Nurse (RN) to work within our clients' homes providing direct patient care and to assist and instruct our patients and their families in proper treatment to improve and maintain their health. This position also assess and record symptoms and/or reactions and progress in patients, administer medications and assist in rehabilitation in accordance with the physician-established plan of treatment. This position holder must possess a majority of patient care skills, good nursing assessment in the level of Home Care setting. Position Requirements Must have a current California RN license Proficiency with electronic health records and web-based applications preferrably WellSky Software. Must be self-directed, able to work independently with little supervision Must be able to travel to prospective patient's place of residence. Preferred to have at least two (2) years prior professional nursing experience in a healthcare setting and one (1) year prior professional nursing experience in Home Health and other healthcare settings

Responsibilities:

Provide treatments and work cooperatively with the members of the team to help meet positive patient care outcomes. Provides direct patient care. Prepares accurate and timely documentation of patient services to reflect the plan of care. To evaluate and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care. Participates in coordination of home health services, appropriately reporting the identified needs to the interdisciplinary group. Examples include Home Aide, OT, PT, SW, ST, or Clinical Supervisor. Uses and prepares equipment and supplies effectively, adhering to aseptic techniques. Wound care experience is preferred. Provides appropriate pain/symptom management. Evaluates patient's response to treatments/medications. Participates in personal, professional growth and development. Also participates in organization's quality assessment performance improvement teams and activities.
