A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Job Description

Hospice of Humboldt is looking for a Full-time Case Manager Nurse for the Palliative Care team. The Case Manager RN is part of a multidisciplinary Palliative Care team providing specialized medical care for people with chronic and/or complex conditions. While focused on community-based palliative care, this role will be cross-trained in hospice care to ensure a seamless transition for patients who may need end-of-life services. Minimum Qualifications Graduate of an accredited nursing program. Current California Registered Nurse license. Minimum of one (1) year of experience as a professional nurse within the previous three (3) years or equivalent experience in a BSN program. Valid driver's license, proof of acceptable automobile insurance coverage, and reliable transportation. Reliable mobile phone with plan that provides good local coverage. Basic computer skills.
