May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.

Experienced Wound Care/Treatment Nurse preferred About the Position Meadowbrook Post Acute is seeking an experienced Wound Care / Treatment Nurse to join our clinical team. This position is ideal for a nurse with strong clinical judgment, excellent wound assessment skills, and hands-on experience managing complex wounds in a skilled nursing or post-acute setting. The Wound Care/Treatment Nurse will be responsible for coordinating and providing comprehensive wound assessment and treatment services while working closely with physicians, nursing staff, residents, families, dietitians, and other members of the interdisciplinary team. We are looking for an experienced professional who can independently assess wounds, recognize changes in condition, follow treatment orders accurately, identify barriers to healing, and ensure appropriate documentation and follow-up. Key Responsibilities Perform comprehensive wound assessments upon admission, readmission, and as clinically indicated. Develop, implement, and monitor individualized wound treatment plans in accordance with physician orders and facility policies. Provide wound treatments and ensure treatments are completed accurately and timely. Manage and monitor pressure injuries, skin tears, surgical wounds, venous/arterial wounds, diabetic wounds, and other complex wounds. Measure and document wound size, drainage, tissue characteristics, wound bed, peri-wound condition, and progression. Monitor wounds for signs and symptoms of infection or deterioration and promptly notify the appropriate provider. Communicate significant wound changes to the physician, Director of Nursing, Administrator, resident/responsible party, and interdisciplinary team as appropriate. Coordinate wound-related physician appointments and specialty consultations when necessary. Collaborate with the MDS, nursing, dietary, rehabilitation, and social services departments to address factors affecting wound healing. Identify nutritional, mobility, continence, pressure-relief, and other risk factors contributing to impaired skin integrity. Ensure appropriate pressure-relieving devices and other interventions are implemented. Review physician treatment orders for accuracy and ensure nursing staff understand and follow current orders. Conduct regular wound rounds and monitor treatment effectiveness. Maintain accurate and timely wound documentation and treatment records. Assist with wound-related care planning and care plan revisions. Monitor compliance with wound-care protocols and facility policies. Provide education and training to nursing staff regarding wound prevention, assessment, treatment, and documentation. Participate in Quality Assurance/Performance Improvement (QAPI) activities related to wounds, pressure injuries, skin integrity, and clinical outcomes. Maintain compliance with applicable CMS, CDPH, Title 22, and facility requirements. Perform other duties as assigned by the Director of Nursing or Administrator.
Qualifications Required:
Current California LVN license in good standing. Previous experience providing wound care in a skilled nursing facility, post-acute, long-term care, hospital, or similar clinical setting. Strong knowledge of wound assessment and treatment principles. Ability to accurately assess and document wound progression. Ability to recognize changes in resident condition and appropriately escalate concerns. Excellent clinical judgment and communication skills. Strong organizational and time-management skills. Ability to work independently while collaborating effectively with the interdisciplinary team.
Preferred:
2+ years of wound-care/treatment experience. Skilled nursing facility experience. Wound Care Certification (WCC), Certified Wound Care Nurse (CWCN), or similar certification. Experience with pressure injury prevention and management. Experience with wound photography/documentation systems. Experience with electronic medical records. Experience educating nursing staff regarding wound care. Experience participating in wound-related QAPI initiatives.
