Duties Conduct detailed patient assessments, including vital signs, medical history review, and physiological evaluations to determine individual care needs. Develop and implement personalized care plans that align with patient goals, medical requirements, and health regulation policies. Manage cases across various settings such as hospital, home health, post-acute care, hospice, and clinic environments to ensure continuity of care. Collaborate closely with healthcare professionals—including physicians, social workers, behavioral health specialists, and discharge planners—to coordinate services effectively. Utilize Electronic Health Records (EHR) systems for accurate documentation of medical records, patient interactions, discharge planning details, and case progress notes. Oversee utilization management processes by evaluating diagnostic evaluations and ensuring appropriate resource use in compliance with healthcare regulations. Provide patient education on health conditions, medication management, infection control practices—including infectious disease care—and self-care strategies to promote optimal recovery. Qualifications Proven experience in critical care environments such as ICU (Intensive Care Unit), Level I or Level II trauma centers, or emergency medicine settings. Strong background in hospital medicine with expertise in medical-surgical nursing and acute care management. Experience working within managed care organizations or familiarity with health regulation policies like HIPAA (Health Insurance Portability and Accountability Act). Skilled in patient service delivery with excellent client communication abilities and the capacity to manage complex cases effectively. Proficiency in EMR/EHR systems for documentation and clinical case management; familiarity with medical terminology is essential. Knowledge of physiology principles and diagnostic evaluation techniques to support comprehensive patient assessments. Ability to work across diverse populations including pediatrics, geriatrics, behavioral health patients, and those requiring post-acute or hospice care. Strong organizational skills for discharge planning, intake procedures, managing hospital records, and coordinating home health services.
