We are seeking a compassionate and dedicated Registered Nurse Case Manager for our Hospice program. This role is essential in providing high-quality patient care and support to individuals and their families during end-of-life transitions. The ideal candidate will possess a strong background in nursing with experience in critical care, geriatrics, and case management. You will be responsible for coordinating patient care, conducting assessments, and ensuring compliance with healthcare regulations. Duties Conduct comprehensive patient assessments to determine care needs and develop individualized care plans. Coordinate with multidisciplinary teams to implement effective patient service strategies. Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes. Provide education and support to patients and families regarding disease processes, treatment options, and end-of-life decisions. Maintain accurate medical records in compliance with HIPAA regulations and organizational policies. Facilitate discharge planning and transition of care from inpatient to outpatient settings. Conduct intake evaluations and diagnostic assessments for new patients. Utilize knowledge of medical terminology, physiology, and nursing practices to deliver high-quality care. Engage in utilization management activities to ensure appropriate resource allocation. Requirements Current Registered Nurse (RN) license in the state of practice. Experience working with Hospice/Terminally ill patients Experience with Hospice Admissions a plus! Excellent communication skills for effective interaction with patients, families, and healthcare team Ability to work independently as well as collaboratively within a team environment.
