Case Manager LVN

Job Description

Case Manager (LVN) Vasona Creek Healthcare Center is seeking a highly organized, compassionate, and proactive Licensed Vocational Nurse (LVN) Case Manager to join our team. In this pivotal role, you will coordinate patient care, manage utilization reviews, and facilitate smooth transitions of care to ensure the highest quality of clinical outcomes for our residents.

$90,000 - $110,000 / year (depending on experience)

Comprehensive package (Medical, Dental, Vision, 401k, Paid Time Off) Key Responsibilities Oversee and coordinate the discharge planning process from admission to transition. Conduct utilization reviews and collaborate with managed care plans and insurance providers for authorizations. Act as the primary liaison between patients, families, physicians, and multidisciplinary care teams. Monitor resource utilization to ensure efficient, cost-effective, and compliant care delivery. Maintain precise documentation of care plans, case notes, and regulatory requirements.
Qualifications License:
Current, unrestricted California Licensed Vocational Nurse (LVN) license.
Experience:
Minimum of 2 years of nursing experience, with a strong preference for prior case management or utilization review experience in skilled nursing or long-term care.
Skills:
Excellent clinical assessment, strong negotiation and insurance authorization skills, and deep knowledge of Medicare/Medicaid guidelines. We are an equal opportunity employer.
