• Acute Care Case Manager with strong experience in care coordination and case management within an acute care hospital setting .

The ideal candidate should have excellent clinical judgment, communication, and organizational skills, with the ability to coordinate patient care across multidisciplinary teams and healthcare services. Travel nursing experience is required and is a hard-stop requirement for this position. Responsibilities Coordinate patient care and develop individualized care plans throughout the acute care stay. Perform patient assessments and identify care coordination, discharge planning, and resource needs. Collaborate with physicians, nurses, social workers, and multidisciplinary teams to ensure continuity of care. Coordinate referrals, follow-up services, home health, DME, and other post-acute care needs. Document care plans, interventions, and patient outcomes accurately in the EMR.
