What You'll Do Lead patient assessments and care planning Manage and update Plans of Care Coordinate IDG meetings Support and supervise LVNs & HHAs Ensure clean, compliant documentation Partner with physicians, patients, and families to deliver exceptional hospice care What You Need 1 year hospice experience (within last 3 years) Active CA RN license Strong communication skills Valid CA driver's license + insurance Current CPR, TB/Physical, and background clearance
