Inland Empire & Orange County Supreme Home Health Agency, Inc. is seeking dependable and compassionate Licensed Vocational Nurses (LVNs) to join our growing home health team on a per-diem basis . We currently have an immediate need for LVNs in: Orange County Inland Empire , primarily Riverside and surrounding areas, with some coverage in San Bernardino County This opportunity is ideal for LVNs looking for flexible, supplemental, or part-time work while providing one-on-one nursing care to patients in their homes. Patient visits are offered based on agency census, geographic needs, and your stated availability. There are no guaranteed minimum hours or patient visits , and per-diem LVNs may accept or decline offered visits based on their availability. Newly licensed LVNs are welcome to apply. Previous nursing and home health experience are preferred but are not required. Position Highlights W-2 employee — not a 1099/independent contractor position $50.00 per completed patient visit Flexible per-diem scheduling Accept or decline offered patient visits based on your availability Opportunities available throughout Orange County and the Inland Empire Mileage reimbursement for eligible travel between patient visits Company-provided tablet for clinical documentation and EVV California Paid Sick Leave in accordance with applicable law and Company policy Opportunity for additional visits as patient census grows Potential opportunity to transition into a full-time position as the agency expands Compensation Per-diem LVNs are compensated $50.00 per completed patient visit . The per-visit compensation structure accounts for the routine work associated with an assigned visit, including: Direct patient care Required clinical documentation Routine travel time between patient locations during the workday Eligible business mileage between patient visits is reimbursed separately. This is a non-exempt W-2 employee position . Compensation and overtime requirements are administered in accordance with applicable California wage-and-hour requirements. Schedule & Availability This position offers significant scheduling flexibility. Patient assignments are offered based on: Your stated availability Patient needs Geographic location Agency census There are no guaranteed minimum hours, shifts, or patient visit volume. You may accept or decline offered patient visits based on your availability. Opportunities may be available weekdays, evenings, and weekends , depending on patient needs and agency census. A maximum of approximately six patient visits per workday is generally scheduled unless additional visits are approved by clinical leadership. Service Areas We are currently recruiting LVNs interested in serving one or more of the following areas: Orange County Inland Empire , including Riverside and portions of San Bernardino County Applicants will have the opportunity to identify their preferred service territory or territories during the application process. Patient assignments are offered based in part on geographic coverage needs and employee availability. Responsibilities As a Home Health LVN, responsibilities may include: Provide skilled nursing care to patients in their homes Perform clinical treatments and nursing services in accordance with physician orders and established Plans of Care Work under the supervision and direction of a Registered Nurse and/or Nurse Practitioner Observe and monitor patient condition and promptly report significant changes to the appropriate clinical team member Obtain and document vital signs and other required clinical information Administer medications and treatments within the LVN scope of practice and in accordance with the patient's Plan of Care Provide patient and caregiver education regarding medications, treatments, safety, and ongoing care Maintain appropriate infection-control and patient-safety standards Complete accurate and timely clinical documentation Utilize the agency's Electronic Health Record (EHR) and Electronic Visit Verification (EVV) systems Communicate effectively with physicians, nurses, patients, caregivers, and other members of the healthcare team Travel between accepted patient assignments Participate in required clinical coordination activities Required Qualifications Current and active California Licensed Vocational Nurse (LVN) license Current CPR/BLS certification Ability to provide safe and professional nursing care in a home health environment Strong communication and organizational skills Ability to work independently while following established Plans of Care and clinical protocols Reliable transportation Valid driver's license and current automobile insurance Ability and willingness to travel between accepted patient locations Ability to use electronic documentation and mobile technology Ability to meet applicable background screening and employment requirements Preferred Qualifications Previous nursing experience is preferred but not required .We especially welcome candidates with:
Previous home health experience Experience providing care to adult or senior patients Experience working independently in a field-based healthcare environment Experience with Electronic Health Record (EHR) or Electronic Visit Verification (EVV) systems Newly licensed LVNs who meet the required qualifications are encouraged to apply.
