Malpractice insurance provided Mileage reimbursed iPad tablet and all supplies provided All appointments scheduled for you What You'll Do Conduct comprehensive in-home wellness assessments Educate members on preventive care benefits Identify potential gaps in care to help reduce avoidable ER visits and hospitalizations Complete documentation using provided EMR tools Visits are face-to-face and take place where members live-allowing for more personalized, impactful care. Requirements Active State NP license and Board Certification Valid NPI Minimum 3 months of wellness exam experience preferred Comfort with EMR/documentation systems Ability to travel within a region (visits are clustered in a 10 mile radius) Bilingual/Spanish Speaking a plus Give on average 2 full days per week.
