Full Job Description Duties:

• Provide compassionate and quality care to patients in hospice settings
• Administer medications and treatments as prescribed by physicians
• Monitor and assess patient's condition and report any changes to the healthcare team
• Assist with pain management and symptom control
• Educate patients and their families on end-of-life care options and provide emotional support
• Coordinate care with other healthcare professionals, including social workers, chaplains, and therapists
• Maintain accurate and up-to-date patient records using electronic health record systems such as Athenahealth or e

ClinicalWorks Experience:

• Minimum of [X] years of experience as a Hospice Nurse
• Proficient in providing care for patients of all ages, including toddlers, neonates, and elderly individuals
• Knowledge of coding and billing practices related to hospice care
• Familiarity with infection control protocols in a healthcare setting
• Experience in providing specialized care for patients with dementia or other cognitive impairments
• Familiarity with the use of neonatal ventilators for respiratory support
