Position Summary We are currently seeking a dedicated LVN for a consistent Sunday overnight case . This is an excellent opportunity for a nurse seeking a stable weekly schedule, supplemental income, 1:1 patient care, and the ability to build meaningful relationships with patients and their families. Nurses interested in additional hours may have the opportunity to pick up extra shifts based on staffing needs and availability. This patient requires ventilator care , therefore applicants must be experienced and certified in ventilator management . Under the supervision of a Registered Nurse, you will provide focused, individualized care while ensuring patient safety and continuity of care. Responsibilities Provide skilled nursing care in the home setting Monitor and manage patient needs Follow individualized care plans and physician orders Monitor, document, and report patient progress and changes in condition Administer medications and treatments as prescribed Collaborate with the interdisciplinary healthcare team Educate patients and families regarding care plans and treatments Maintain accurate and timely clinical documentation Qualifications Current and active California LVN license Current BLS certification Ventilator experience required Must be trained and comfortable caring for ventilator-dependent patients G-Tube and Trach experience required Current TB test or chest X-ray (if applicable) Valid driver's license and reliable transportation Strong communication and documentation skills Compassionate, dependable, and professional demeanor Schedule Sunday Overnight 10:00 PM to 6:00 AM (2200-0600) Consistent weekly schedule Long-term case assignment available Why Join Prime Home Health?
