This position plays a critical role in coordinating the Resident Assessment Instrument process, supporting accurate reimbursement, maintaining regulatory compliance, and developing individualized resident care plans. The ideal candidate is a California-licensed LVN or RN with MDS experience, strong clinical judgment, excellent attention to detail, and the ability to work effectively with an interdisciplinary team. Key Responsibilities Coordinate and complete MDS assessments, Care Area Assessments, and related documentation in accordance with federal and California requirements Maintain an accurate assessment schedule and ensure all assessments are completed and submitted by required deadlines Establish and track Assessment Reference Dates, Medicare assessment dates, quarterly assessments, annual assessments, significant change assessments, and discharge assessments Accurately code the MDS using information obtained through medical record review, resident observation, staff interviews, and communication with residents and families Review clinical documentation to ensure it supports MDS coding, resident acuity, skilled services, case-mix classification, and reimbursement Collaborate with nursing, rehabilitation, social services, dietary, activities, and other departments throughout the MDS and care-planning process Coordinate interdisciplinary care-plan development and updates based on each resident's individual needs, preferences, risks, and goals Participate in care-plan conferences, Medicare meetings, daily clinical meetings, and other interdisciplinary meetings as needed Monitor documentation for accuracy, completeness, timeliness, and compliance with Medicare, Medicaid, PDPM, RAI, OBRA, and California regulatory requirements Identify documentation or coding concerns and communicate needed corrections to the appropriate department or team member Provide education and guidance to facility staff regarding documentation standards and MDS requirements Assist with regulatory surveys, audits, quality-improvement activities, and reimbursement reviews Maintain current knowledge of MDS, RAI, PDPM, Medicare, Medicaid, and other regulatory or reimbursement changes Protect resident confidentiality and comply with all facility policies, procedures, and professional standards Perform other duties related to resident assessment, care planning, and clinical reimbursement as assigned Qualifications Current and active California LVN or RN license in good standing At least one year of experience working as an MDS Nurse or MDS Coordinator Skilled nursing or long-term care experience required Working knowledge of the MDS 3.0 and RAI processes Knowledge of PDPM, Medicare reimbursement, case-mix principles, OBRA requirements, and the care-planning process Experience using PointClickCare preferred Strong medical-record review and clinical documentation skills Excellent organizational, time-management, and attention-to-detail skills Ability to manage multiple assessment deadlines and priorities Strong written and verbal communication skills Ability to work cooperatively with residents, families, nursing staff, facility leadership, and the interdisciplinary team Current CPR certification preferred Physical Requirements Ability to sit, stand, and walk throughout the workday Ability to perform prolonged computer and documentation work Ability to occasionally lift or move up to 25 pounds Cornerstone Care Center is an equal opportunity employer.
