MDS Coordinator Valley View Post Acute - 3.7 South El Monte, CA Job Details Full-time From $40 an hour 1 day ago Qualifications Collaborate with healthcare professionals Care documentation MDS Schedule management Overseeing care coordination Full Job Description Manage and maintain the MDS/PPS program, assessment schedule and tracking to ensure timely completion of assessments by all departments. Develop and coordinate nursing care plans based on individualized patient needs and monitor to ensure other depatments are timely completing care plans. Analyze assessment data to identify complex patient needs and early clinical changes. Evaluate the effectiveness of care plans and update the MDS/RAI process within regulatory timelines. Collaborate with interdisciplinary teams to ensure comprehensive and compliant documentation. Maintain compliance with all applicable regulations and facility policies. Perform other duties as assigned to support resident care and facility operations.
