Temple City Adult Day Health Care (ADHC) is a community-based program providing a variety of medical, rehabilitative, therapeutic and recreational activities and services for older and/or disabled adults.

We are looking for a qualified RN to fill an open position at the center.

Duties and Responsibilities:

Works with Medical Director and participating physicians and therapist to ensure appropriate care of program participants. Writes patient Care Plans Oversees all medical staff and provides select nursing care. Participates in quarterly Utilization Review Conferences Oversees all medical staff and provides select In-Service Training of staff. Reports to Program Director on a regular basis. Monitors status and progress of program participants and recommends appropriate levels of care for those participants with needs unable to be met in an ADHC setting. Assists in all reports and proposal writing. Conducts initial assessments and reassessments to determine skilled nursing needs and need for personal care and assistance in activities of daily living. Provided skilled supportive and restorative care. Acts as a liaison with participant's physician. Provides health education and counseling to participants and their families, if indicated Responsible for the administration of medications and training in self- medication Daily charting for routine services may be done with a flow chart which can be checked off and initialed. Defines the responsibility of licensed nursing staff and program aides; also provides supervision. Acts as a member of the multidisciplinary team and participates in all case conferences. Responsible for the health areas of orientation and in services training for staff and volunteers; participates in monthly sessions. Provides evaluation to determine of emergency medical service required.

Qualifications:

RN licensed by Calif. State Ability to establish relationships with community leaders, the public in general and the elderly specifically. Write and speak fluent

English Bilingual:

Mandarin or Cantonese Demonstrate an understanding of and compassion toward the elderly.

Experience:

Preferably with experience in

ADHC/CBAS

setting
