Overview The Clinic Registered Nurse works under the Clinic Manager to support efficient patient flow, safe, high-quality clinical care, care coordination, and clinic operations across both the Family Practice/Rural Health Clinic and the Specialty Clinic. The RN functions as a member of the interdisciplinary health care team, collaborating with physicians, NP/PAs, medical assistants, patient service representatives, medical records, health educators, specialty providers, and other professionals to provide comprehensive, family-centered, non-judgmental care across the lifespan. The RN also supports specialty clinic workflows, procedures, patient education, follow-up, and care coordination as assigned. Assist with daily back-office clinic operations, including patient flow, Medical Assistant support/staffing, visit productivity, billing support, EHR documentation, and the overall patient experience. Perform telephone and in-person triage; assess patient needs, prioritize care, gather pertinent histories and clinical information, and communicate findings to the appropriate provider. Provide direct nursing care, including nurse visits, medication administration, immunizations, vital signs, monitoring of patient conditions, and assistance with minor procedures within RN scope of practice. Provide patient and family education regarding preventive care, healthy lifestyles, disease management, nutrition, exercise, medication adherence, and provider-directed treatment plans. Participate in chronic disease management for patients with conditions such as diabetes, hypertension, asthma, and other chronic or complex medical conditions; support care plans and monitor outcomes as directed. Coordinate referrals, follow-up appointments, and transitions between care settings to promote continuity of care. Accurately document assessments, interventions, patient education, communications, nurse visits, and charge-entry activities in the medical record. Collaborate with physicians, advanced practice providers, medical assistants, social workers, patient access staff, and other professionals to develop and implement comprehensive treatment plans. Support family-centered care by recognizing patient and family needs, risk factors, barriers to care, and social determinants of health and communicating concerns to the care team. Handle provider and pharmacy calls and follow established protocols for medication-related communications and refill requests. Support clinic compliance with applicable programs and requirements, including VFC, OSHA, RHC, HIPAA, infection prevention, medication management, patient safety, and facility policies. Assist with quality reporting requirements, including preventive services, chronic disease management, regulatory compliance, and other clinic quality measures. Support Specialty Clinic needs as assigned. Support the St. Vincent Health employee health program as assigned, including triage evaluation of work-related injuries and illnesses, pre-placement/post-offer assessments, health surveillance, assistance with DOT examinations, and return-to-work assessments. Maintain documentation required by OSHA and facility policies and procedures; apply knowledge of health promotion, workplace hazards, ergonomics, workers' compensation, and OSHA standards. Support infection prevention activities for clinic operations and, when assigned, organizational infection prevention responsibilities. Exhibit a caring and professional attitude toward patients, visitors, and co-workers and contribute to a positive, team-oriented work environment. Participate in required training, in-services, meetings, annual competencies, and professional development activities. Plan and organize work to complete assignments on time, request assistance or guidance when necessary, and accept additional tasks as assigned. Follow the chain of command, attendance expectations, standard precautions, emergency/disaster procedures, and all facility policies and procedures. Complete occurrence reports and other required documentation in accordance with facility policy. Support Specialty Clinic operations and patient care needs, including rooming support, clinical preparation, medication reconciliation, patient education, follow-up, and coordination with specialty providers. Coordinate specialty referrals, diagnostic testing, procedures, outside records, prior clinical information, and follow-up appointments to promote continuity of care. Assist specialty providers with clinic-based procedures and treatments within RN scope of practice and demonstrated competency. Provide pre-procedure and post-procedure patient education and instructions as directed by the specialty provider. Communicate specialty provider recommendations, test results, medication instructions, and follow-up plans to patients in accordance with established protocols and scope of practice. Collaborate with Family Practice, Specialty Clinic, hospital departments, and outside organizations to support smooth transitions of care and timely completion of specialty care plans. Qualifications Graduate of an accredited Registered Nursing program; an Associate Degree in Nursing is required Bachelor's degree in Nursing (BSN) or equivalent preferred Minimum of two years of clinical experience in a health care setting required, demonstrating progressive responsibility and peer mentoring ability Experience in family practice, primary care, rural health, ambulatory care, or a highly service-oriented environment preferred Experience working with multiple service lines and using metrics or measurement tools to assess results preferred Occupational health experience recommended Current Colorado RN license or current RN license from a participating Nurse Licensure Compact state. BLS certification required PALS certification required Certified Occupational Health Nurse (COHN or COHN-S) preferred Ability to communicate clearly and maintain effective working relationships with patients, families, staff, physicians, and allied health professionals Ability to perform phlebotomy and basic laboratory functions Demonstrated knowledge of growth and development across the lifespan and ability to interpret information related to age-specific patient needs Strong assessment, triage, patient education, care coordination, documentation, organizational, and time management skills Ability to manage multiple priorities, work independently, and collaborate effectively within an interdisciplinary team Bilingual skills preferred Join us to make a meaningful difference in patients' lives through compassionate nursing care!
