Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage plan participants Review prior and current medical history, chronic conditions, and medication usage Educate members on their health conditions and medication regimens Identify gaps in care and advocate for follow-up or additional resources Document health assessmentsno treatment planning or prescribing required

Required Qualifications:

Current Nurse Practitioner license in Colorado Active CPR certification (BLS required) Board certification through AANP or ANCC Completion of an MSN from an accredited nursing school At least 6 months of recent nurse practitioner experience is preferred This position rewards your expertise with a competitive evaluation fee ranging from $80 to $90 per completed risk assessment, in addition to a mileage stipend and compensation for administrative time.
