Job Description

Job Summary We are seeking a dedicated and detail-oriented MDS (Minimum Data Set) Coordinator to join our healthcare team. In this vital role, you will oversee the comprehensive assessment and documentation processes required for long-term care residents, ensuring compliance with federal and state regulations, including Medicare and Medicaid standards. Your expertise will support accurate clinical documentation, facilitate effective case management, and optimize reimbursement through precise coding and utilization review. This position offers an exciting opportunity to impact patient care quality while working within a dynamic healthcare environment that values professionalism, accuracy, and proactive communication. Responsibilities Coordinate and complete the MDS assessments for long-term care residents in accordance with CMS (Centers for Medicare and Medicaid Services) guidelines, ensuring timely submission. Review medical records, clinical documentation, and physician notes to ensure accuracy and completeness in compliance with HIPAA and health regulation policies. Collaborate with interdisciplinary teams—including nursing, therapy, social work, and physicians—to gather necessary information for comprehensive assessments. Utilize EMR (Electronic Medical Records) systems such as Cerner, Epic, or Athenahealth to document resident data accurately and efficiently. Conduct documentation reviews to ensure adherence to clinical documentation standards and coding accuracy using
ICD-10/ICD-9
codes, CPT codes, DRG (Diagnosis-Related Group), and other medical coding practices. Perform utilization management by analyzing resident needs against services provided to optimize care plans while maintaining regulatory compliance. Stay current on long-term care regulations, Medicare/Medicaid policies, NCQA standards, and CMS updates to ensure ongoing compliance. Assist in discharge planning, case management activities, and coordination of hospice or home care services as needed. Support quality improvement initiatives by participating in audits related to medical records, coding accuracy, and documentation standards. Experience Proven experience in long-term care facilities such as nursing homes or skilled nursing facilities with a strong understanding of Medicare regulations and CMS guidelines. Prior experience with inpatient or outpatient healthcare settings that involved clinical documentation improvement or utilization review. Familiarity with EMR/EHR systems including Cerner, Epic, eClinicalWorks, or Athenahealth is highly preferred. Knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-10/ICD-9), CPT coding, DRG assignment, and healthcare reimbursement processes. Demonstrated ability to manage medical records efficiently while ensuring compliance with HIPAA and state healthcare regulations. Experience working within hospital settings such as ICU or trauma centers is advantageous; familiarity with acute care protocols enhances effectiveness. Strong understanding of health information management principles related to patient care documentation review and medical management strategies.
