Full Job Description Position Summary:
The primary purpose of the registered nurse (RN) is to provide nursing care and assistance, care coordination, medication adherence monitoring, nursing education, medication administration, and oversight of day-to-day clinical duties and functions.
Responsibilities:
Interview patients, measure vital signs, and record patient information. Work with staff, internal/external medical providers, and patients to reach healthcare goals and provide excellent quality of care through care coordination. Facilitate patient access to appropriate medical and specialty providers or services Monitor adherence to care plans, evaluate effectiveness, monitor patient progress in a timely manner, and facilitate changes as needed Provide nursing education to patients as directed by provider(s) Administer medications as per provider's orders Provides infection prevention and control monitoring and education Manages orders and maintains logs of medications and supply stock and inventory Monitor laboratory and other test results to be reviewed by provider, as needed. Assist in preparing reports and statistical data, as needed. Complete staff competency training Ensure quality control measures are met with clinical tests and equipment Engage in all offered training, consultations and supervisions made available Assist with policy and procedure development Follow and fully implement agency polices Fully and professionally respond to and implement directives and advice offered by all supervisory and training personnel, as well as professionally offered feedback from coworkers Perform other duties as assigned Qualifications (Required): Graduate of an accredited school of nursing program Valid license in CT Demonstrated ability to utilize computer technology and to willingness to learn Competency in nursing practice and clinical management Knowledge of medical terminology Qualifications (Preferred): Experience with HIV and sexual health patient care Minimum of 2-4 years related experience and/or training. Bilingual (Spanish) is strongly preferred.
Pay:
$38.00 - $45.00 per hour Expected hours: 30.0 - 37.5 per week
