Full-Time Baylor Schedule Enjoy Full-Time Pay Working Just 3 Days Per Week We are seeking an experienced Home Health Registered Nurse (RN) to join our growing team in the Hamden area. This Baylor position offers a flexible weekend schedule with full-time salary, bonus potential, and comprehensive benefits while allowing you to enjoy four days off each week. Many of our weekend nurses find that a schedule focused on Start of Care visits and admissions creates shorter, highly productive workdays while still making a meaningful impact on patients and families. Responsibilities Complete Start of Care (SOC), Resumption of Care (ROC), and other skilled nursing visits. Perform comprehensive patient assessments in patients' homes. Accurately complete OASIS documentation. Develop and coordinate individualized care plans. Educate patients and caregivers regarding diagnoses, medications, and treatment plans. Collaborate with physicians and interdisciplinary team members to ensure quality patient outcomes. Maintain timely, accurate, and compliant clinical documentation. Qualifications Active Connecticut RN license in good standing. Minimum one year of home health experience preferred. OASIS proficiency required. Experience completing Start of Care assessments strongly preferred. Strong clinical assessment, documentation, and time-management skills. Valid driver's license and reliable transportation.
