Job Overview Staff Mates Homecare is family-owned and has been in business for over 40 years. We are seeking a compassionate and skilled Home Care RN to provide high-quality healthcare services to patients in their homes. This role requires a commitment to patient-centered care, ensuring that each individual receives the support they need to maintain their health and well-being. Duties Deliver personalized nursing care to patients in their home environment. Administer medications and monitor patient responses to treatments. Provide health coaching and education to patients and their families regarding health management. Assist with case management, coordinating care plans with other healthcare professionals. Conduct assessments for patients requiring specialized care, including infant care and internal medicine needs. Maintain accurate patient records and documentation of care provided. Experience Previous experience in home community care or similar nursing roles is preferred. Experience in medication administration and case management is essential. Knowledge of health coaching techniques to empower patients in managing their health effectively.
