We are currently looking for a compassionate clinical care provider to join our team! The successful candidate will hold a current CT license as an RN, APRN or PA and be able to administer vaccines but that is not all they will do! Our new Public Health Nurse will provide support for a wide variety of LLHD programs including lead poisoning prevention, communicable disease control, public health emergency preparedness, chronic disease prevention and more. They will work in a variety of settings and with a range of community members to support overall health and wellbeing for people throughout our community. This is an exciting opportunity for someone to mix clinical care with program administration with community outreach and education; never be bored because no two days look alike; and maintain a regular daytime/weekday work schedule. Duties and Responsibilities:

(Examples, not a comprehensive listing): Administer vaccines at the office, in community-based settings and for homebound community members. Conduct health screenings in community-based settings. Support District community-based care initiatives. Support and provide clinical expertise to public health emergency response activities. Follow response and follow-up protocols for various public health concerns including but not limited to tuberculosis, vaccination schedule compliance, foodborne illness, elevated blood lead levels and emerging public health concerns. Assure District compliance with Connecticut Vaccine Program requirements and protocols. Complete and maintain required documentation, forms, data entry and reports for assigned tasks. Support vaccination providers in adhering to Connecticut Vaccine Program requirements and protocols. Provide public health nursing support for all District programs. Provide health education and information to community members, community partner organizations and clinical providers.

Written and oral fluency in English required; writing and oral fluency in Spanish or Haitian Creole preferred. Understanding of public health, health equity, social/structural determinants of health and health behavior/decision making. Ability to lift up to 50 lbs. Valid CT Driver's License and ability to travel to assigned locations in District cars.

Pay:

$34.00 - $40.00 per hour Expected hours: 32.0 - 40.0 per week
