2996663325-2 Apply for this job now Job Description Hartford Healthcare seeks an On Call Visiting Nurse to provide after-hours home visits and triage for patients across our network. In this field-based role, you will perform comprehensive assessments, administer medications and treatments, and collaborate with physicians to manage acute changes in condition. You will document care in our EMR, educate patients and families, and ensure safe, compassionate, high-quality service. Responsibilities Provide on-call home visits for acute patient needs within assigned territory Conduct comprehensive nursing assessments and deliver skilled care interventions Administer medications, perform wound care, and manage IV therapies per protocols Collaborate with physicians and interdisciplinary team to update care plans Document all care accurately and timely in the electronic medical record Educate patients and families on disease management, medications, and safety Prioritize and triage incoming calls to determine appropriate level of response Participate in quality and safety initiatives to improve patient outcomes Maintain infection control and regulatory compliance in the home setting Support Hartford Healthcare's culture of compassion, teamwork, and excellence Required Skills Patient assessment IV therapy and medication administration Wound care management Care planning and documentation (EMR) Home health nursing protocols Triage and clinical decision-making Infection control Patient and family education Care coordination with physicians Time management and autonomy
