2996663290-2 Apply for this job now Job Description Hartford Healthcare, a leading not-for-profit healthcare system in Connecticut, is seeking a Revisit Nurse to join our Healthcare & Medical Services team. In this role, you will conduct follow-up assessments, review care plans, coordinate with multidisciplinary teams, and support patients transitioning between care settings. You'll ensure continuity of care, reinforce education, and identify barriers to recovery. Responsibilities Conduct follow-up nursing assessments for recently discharged patients Monitor patient symptoms, vitals, and response to treatment Update and implement individualized care plans in collaboration with the care team Provide patient and family education on medications, symptom management, and self-care Coordinate care with physicians, therapists, social workers, and case managers Document all encounters accurately in the electronic medical record Identify early signs of deterioration and escalate care as needed Support safe transitions of care and help prevent avoidable readmissions Participate in quality improvement and patient safety initiatives Maintain compliance with regulatory and infection control standards Required Skills Patient assessment Care planning Clinical documentation Electronic Medical Records (EMR) usage Medication management Discharge and transition-of-care planning Patient and family education Care coordination and case management Vital signs monitoring and symptom triage Infection control practices
