Job Summary We are seeking a dedicated and highly skilled Registered Nurse to provide comprehensive patient care across diverse healthcare settings. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and medical history documentation using advanced EMR (Electronic Medical Records) systems such as Point Click Care. Administer medications and injections accurately following established protocols, medication administration. Monitor patients' physiological status in various settings including long-term care and hospice environments. Collaborate with multidisciplinary teams to develop individualized care plans aligned with HIPAA regulations and NCQA standards. Educate patients and their families on health coaching, medication management, and chronic disease management including geriatrics and behavioral health. Ensure strict adherence to infection control protocols and maintain accurate medical documentation for legal compliance and quality assurance purposes. Skills Proficiency in EMR/EHR systems such as Point Click Care. Skilled Care and Long Term Care experience . Strong knowledge of anatomy and physiology to support accurate patient assessment and diagnostic evaluation. Expertise in managing diverse patient populations including geriatrics, individuals with disabilities or developmental disabilities, memory care patients, and those requiring hospice or palliative medicine services. Excellent communication skills for effective collaboration with healthcare teams; capable of working with multidisciplinary professionals across various specialties including occupational health and family planning.
