Position Summary We are seeking a dedicated and compassionate Advanced Practice Registered Nurse (APRN) to join our growing team. This role offers a unique blend of clinical environments—providing continuity of care across SNFs, ALFs, IPRs and our modern outpatient clinics. The ideal APRN candidate will be a motivated self-starter with strong clinical and communication skills, capable of delivering high-quality care to a diverse adult and geriatric population. Key Responsibilities Provide comprehensive primary care to patients in Skilled Nursing Facilities, Assisted Living Facilities, In-Patient Rehabs and clinic settings Perform physical exams, manage chronic and acute conditions, order and interpret diagnostic tests Collaborate with physicians, care teams, and facility staff to coordinate and optimize patient care Document all patient encounters accurately and timely in the electronic medical record (EMR) system Participate in case management, patient education, and transitional care planning Maintain compliance with all regulatory, clinical, and organizational standards Qualifications Current and unrestricted Florida APRN license Certification as an Adult, Family, or Geriatric Nurse Practitioner (ANP, FNP, or AGNP) Minimum of 1-2 years of experience in primary care, geriatrics, or post-acute care preferred DEA license or eligibility Strong interpersonal and communication skills Ability to work independently and manage a flexible schedule across multiple care settings Proficiency with EMR systems What We Offer Competitive salary and productivity incentives Health, dental, and vision insurance Continuing education support and licensure assistance Supportive team environment and strong administrative infrastructure Opportunity to make a real impact on the health of our senior community
