Position Summary:

The Registered Nurse (RN) - Home Health provides skilled nursing care to patients in their homes, focusing on health promotion, disease management, and recovery. The RN assesses patient conditions, develops and implements individualized care plans, and collaborates with physicians and interdisciplinary teams to ensure optimal patient outcomes.

Key Responsibility:

Assess patient health status and perform comprehensive nursing evaluations in the home setting. Develop, implement, and update individualized care plans in collaboration with physicians and the care team. Administer medications, treatments, and wound care as prescribed.

Requirements:

Active, unrestricted RN- FL State license/ Compact License 1 year as RN Home Health in Skilled home care experience required Must have home health experience with OASIS training and experience . BLS by

AHA COVID

card (Medical/Religious exemption are accepted)
