Job Description Job Description The Lakes Home Care is seeking a licensed Nurse Practitioner (APRN) to support our home health operations. The NP will play a key role in ensuring compliance with Medicare guidelines, assisting with Plan of Care (POC) documentation, and conducting Face-to-Face patient encounters.
Responsibilities:
- Review and sign Plan of Care (POC) and recertifications
- Perform Face-to-Face encounters as required
- Support clinical documentation compliance (Medicare/AHCA)
- Collaborate with RNs, MDs, and administrative staff
- Assist in reducing hospital readmissions through patient follow-up
Requirements:
- Active Florida APRN license
- Active NPI number
- Medicare enrollment (PECOS) preferred
- Experience in Home Health or geriatrics preferred
- Knowledge of Medicare guidelines and documentation
Compensation:
- Per visit: (depending on service)
- POC review/signature: negotiable
- Monthly contract option available
