This role involves managing patient care to ensure optimal outcomes, effective resource utilization, and quality service delivery. This position is ideal for individuals looking to work on a flexible schedule while contributing to our mission of care and service improvement. Responsibilities Assess new patient cases and develop effective care plans in accordance with their health needs. Coordinate with healthcare professionals to ensure that care plans are implemented effectively. Monitor patient progress and adjust care plans as necessary. Educate patients and their families on disease processes, treatment, and home care strategies. Ensure compliance with all healthcare regulations and standards. Required Skills Strong clinical skills in nursing Excellent communication and interpersonal skills Proficient in healthcare management systems Ability to work independently and in team settings Strong organizational and problem-solving skills
