Job Overview The Registered Nurse (RN) provides skilled nursing services to patients in their place of residence in accordance with physician orders, professional nursing standards, state and federal regulations, and CHAP accreditation requirements. The RN functions as the case manager for patients receiving nursing services, collaborates with physicians and the interdisciplinary team, and teaches and educates patients and their families to promote optimal health outcomes and independence. Essential Duties and Responsibilities

• Serves as case manager for all patients receiving nursing services or combined nursing and therapy services.
• Maintains clinical records for all patients- is familiar with OASIS
• Provides skilled nursing services withing the scope of practice and physician orders
• Obtains and documents comprehensive, medical, functional and psychosocial histories
• Conducts home safety checks and environmental assessment
• Performs initial, recertification and follow-up assessments, completing all documentation within 24 hours in the electronic medical record (EMR)
• Develops implements and revises individualized Plan of Care in collaboration with physicians and interdisciplinary team.
• Evaluates ADLs/IADLs to determine need for supportive services
• Determines medical necessity for additional services and initiate referrals
• Assigns patient care tasks to LPNs and HHAs while retaining responsibility for patient outcomes.
• Supervises LPNs and HHAs per state, federal and accreditation requirements, documenting supervisory visits.
• Adjust frequencies of professional services based on patient condition and outcomes
• Notifies physician promptly of change or deviations from POC
• Reviews billing processes with patients/families when applicable
• Communicates effectively with physicians, patients, families and interdisciplinary team regarding POC and patient progress
• Participates in interdisciplinary case conferences, in-service education, training classes and staff meetings
• Participates in Quality Assurance (QA) and Performance Improvement (PI) programs
• Performs clinical record reviews (CRR) as required
• Precepts and mentors newly hired nursing staff s requested
• Maintains compliance with CHAP, Medicare Conditions of Participation, Confidentiality (HIPAA) and agency polices
• Demonstrate professionalism, accountability, ethical conduct, and cultural sensitivity
• Ensures fiscal and clinical appropriateness of all visits that supports medical necessity and skilled services
• Notifies agencies promptly of emergencies, illness or circumstances affecting care or caseload.
• Complies with mandatory reporting laws and incident reporting requirements
• Maintains ethical behavior and professional boundaries.

Experience Proven experience in home health nursing or medical-surgical experience with a strong foundation in patient assessment and medical documentation. Familiarity with advanced nursing procedures including IV insertion, Piccline care/wound care & laboratory specimen collection and aseptic technique application Experience working with individuals across age groups Strong understanding of infection control practices and OSHA standards to ensure safe home environments for patients and staff alike. QUALIFICATIONS

The RN must meet all credentialing and regulatory requirements: Active and unrestricted RN license to practice professional nursing in the state of Florida Minimum of one (1) year experience as a professional nurse; home health experience preferred Bachelor of Arts/Science in Nursing from a program approved by the National League for Nursing preferred Successful completion of a criminal background check per AHCA standards Current CPR certification Valid Driver's License and automobile insurance. Maintains compliance with CHAP standards, Medicare Conditions of Participation, HIPAA, Medicaid, APD and agency documentation policies
