Job Description
Job Summary Join our dedicated home health care team as a Home Health Nurse providing overnight care, ensuring patients receive compassionate, high-quality medical attention in the comfort of their homes. This dynamic role offers an opportunity to make a meaningful difference in patients' lives while working overnight shifts that fit your lifestyle. As a vital member of our healthcare team, you will deliver comprehensive nursing services, coordinate care plans, and support patients with diverse medical needs, including those with complex chronic conditions and disabilities. Your expertise will help promote safety, independence, and well-being for our clients during overnight hours. Schedule - 10pm-10am 32-40 hours per week Duties Perform thorough patient assessments, including vital signs, physical examinations, and medication reconciliation to ensure optimal care delivery during overnight shifts. Administer medications and injections accurately following physician orders, including IV infusions and other advanced therapies. Manage airway care, catheterizations, wound dressings, and sterile procedures with precision and adherence to infection control protocols. Monitor patients' conditions closely, recognize signs of deterioration such as acute pain or infectious disease symptoms, and escalate concerns promptly. Document all patient interactions meticulously using Electronic Health Records (EHR) systems like Epic or Cerner while maintaining strict compliance with HIPAA confidentiality policies. Educate patients and their families on disease management strategies such as dementia care, Alzheimer's support, or long-term care plans to foster independence. Collaborate with interdisciplinary teams including physicians, case managers, and therapists to coordinate discharge planning and post-acute care services. Experience Proven experience working as a registered nurse in settings such as home care, skilled nursing facilities, hospitals (including ICU or trauma centers), or emergency rooms. Critical care experience preferred, especially ICU or Level I Trauma Center exposure demonstrating advanced patient assessment skills. Familiarity with EMR/EHR systems such as Epic or Cerner for accurate documentation and case management. Hands-on experience with IV infusion therapy, medication administration including ICD coding (ICD-9/10), and sterile processing techniques. Knowledge of long-term care regulations, dementia and Alzheimer's care practices, pain management strategies, and infectious disease protocols. Strong background in patient assessment techniques across diverse populations including geriatric patients and individuals with disabilities or developmental delays. Ability to handle triage situations effectively in urgent or emergency scenarios while maintaining clinical confidentiality at all times.
