PM Key Responsibilities:

Conduct thorough OASIS assessments and develop personalized care plans. Deliver direct nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management. Track patient progress, recognize significant health changes, and communicate promptly with the broader care team and physicians. Provide comprehensive education to patients and families about conditions, treatments, and preventive strategies. Accurately document all patient encounters and updates using Home Care Home Base (HCHB). Work collaboratively with interdisciplinary team membersphysical therapists, occupational therapists, social workers, and aidesto ensure holistic, coordinated care. Participate in ongoing staff meetings, case conferences, and professional development activities. Uphold all clinical, regulatory, and agency standards during service delivery and documentation.

Required Qualifications:

Current RN license (in good standing) in the state of practice At least one year of recent home health nursing experience, with field-based care preferred Proficiency with Home Care Home Base (HCHB) and OASIS assessments Reliable transportation, valid drivers license, and current auto insurance
