You'll work closely with a diverse population—from newborns to seniors—ensuring they receive personalized, high-quality care. Your expertise in advocacy, communication, education, and care coordination will help patients navigate their health challenges with confidence. What You'll Do As part of our dedicated team, your responsibilities will include: Developing personalized care plans to support patients in overcoming health challenges Offering education and support to enhance patient safety and adherence to treatment plans Conducting telephonic and in-person assessments to gather key health data Collaborating with physicians, caregivers, and healthcare professionals to provide the best possible care Helping patients understand their benefits and available resources Monitoring and evaluating care plans to ensure positive health outcomes Performing other duties as assigned to support case management initiatives and enhance patient care You'll also play a key role in identifying trends and opportunities to enhance healthcare programs, working alongside physicians and medical directors to implement meaningful changes. Key qualifications include:

RN or LPN license in good standing (FL state) Experience in case management, hospital care, SNF, or health plan settings preferred Strong communication and organizational skills Ability to multitask in a fast-paced environment
