Registered Nurse (RN) Case Manager - Skilled Services FirstLight Home Care is seeking a compassionate, experienced, and detail-oriented Registered Nurse (RN) Case Manager to oversee skilled nursing services within an assigned territory. The RN Case Manager will play a key role in ensuring high-quality client care by conducting in-home assessments, developing and updating care plans, coordinating skilled services, supervising nursing personnel, and collaborating with clients, families, physicians, and the care team. This is an excellent opportunity for an RN who enjoys both clinical care and case management and is committed to providing exceptional home health services. Key Responsibilities Participate in office and field operations to ensure quality assurance and excellence in skilled services. Conduct initial and ongoing in-home nursing evaluations and develop individualized care plans. Coordinate and oversee skilled nursing services based on each client's needs. Collaborate with schedulers to match nurse qualifications, skills, and availability with client needs. Complete all required visit documentation and records accurately, thoroughly, and within established timeframes. Conduct client visits when there are new wounds, changes in condition, or other clinical concerns requiring assessment. Update care plans and communicate changes to the appropriate care team members. Complete comprehensive chart reviews for assigned clients at least every 90 days. Assess skilled clients promptly following hospital discharge, including medication reconciliation and care-plan updates. Obtain and maintain appropriate physician/medical orders and modify care plans as needed. Complete initial, three-year, and as-needed competency assessments for nursing staff. Provide clinical input for nurse performance evaluations and appraisals. Participate in the hiring and onboarding process for nurses as needed. Participate in quality assurance reviews and agency service evaluations. Promote continuous quality improvement throughout skilled services. Assist with developing and reviewing agency policies and procedures to support compliance with AHCA, The Joint Commission (TJC), and current best-practice standards. Stay current with agency technology, software, and documentation systems. In the event of a last-minute staffing vacancy, provide necessary skilled nursing care to clients when needed. Maintain compliance with all FirstLight Home Care policies, procedures, and applicable regulations. Perform other duties as assigned or required. Supervisory Responsibilities The RN Case Manager has direct supervisory responsibility for nursing personnel, including clinical oversight, competency evaluation, care coordination, and performance input. Qualifications Current, unrestricted Registered Nurse (RN) license in the applicable state. Nursing degree from an accredited nursing program. Previous home health, home care, case management, or supervisory nursing experience preferred. Strong clinical assessment and care-planning skills. Excellent communication, organization, and time-management skills. Ability to work independently while effectively collaborating with clients, families, physicians, nurses, schedulers, and other members of the care team. Strong attention to detail and accurate clinical documentation skills. Knowledge of applicable home care regulations, quality assurance standards, and best practices. Reliable transportation and the ability to travel to client homes within the assigned territory. Comfortable using electronic documentation and scheduling software.
