Position Overview We are seeking a compassionate and experienced Registered Nurse (RN) Care Coordinator to support customers through a relationship-centered, Direct-to-Consumer (DTC) care model. In this role, the RN will provide personalized care management, health coaching, and care coordination to help individuals optimize their health, manage chronic conditions, and navigate their personal healthcare journey more effectively. This position works collaboratively with Morley team members and interdisciplinary providers while maintaining the autonomy of professional nursing practice.

•This role is onsite 5 days a week in our Alpharetta office. Work hours are 9:00am to 5:00pm EST. Key Responsibilities Clinical Assessment & Care Planning Conduct comprehensive biopsychosocial and functional health assessments Perform medication reconciliation and identify adherence or safety concerns Identify care gaps and risk factors Develop individualized care management plans Escalate clinical concerns to customers or their outside clinical team or emergency services when appropriate Health Coaching & Patient Education Utilize motivational interviewing to support sustainable behavior change Provide education on chronic disease self-management including: Diabetes Hypertension Obesity Autoimmune conditions Promote preventive care and wellness strategies including nutrition, physical activity, stress management, and sleep Care Coordination, Monitoring & Ongoing Engagement Support care transitions such as hospital-to-home or post-surgical recovery Connect clients with community resources when appropriate Review client-reported metrics such as but not limited to blood pressure, glucose logs, weight trends, and symptom tracking Monitor health changes and escalate concerns appropriately Maintain regular client engagement via telehealth, phone, secure messaging etc. Documentation & Compliance Document assessments, care plans, education, and client interactions in the designated platform Maintain HIPAA compliance and data privacy standards Adhere to all quality nursing measures and organizational policies Qualifications Active, unrestricted Registered Nurse (RN) license 3+ years of clinical experience (primary care, case management, chronic care, concierge medicine, or population health preferred) Experience supporting chronic disease management Strong communication and telehealth engagement skills Training in motivational interviewing or health coaching preferred Electronic Health Record (EHR) experience
