We're currently hiring for a Visiting Nurse to provide mobile primary care in the Blue Ridge area .This is a full-time opportunity with flexibility. Our mobile approach allows you to provide care directly in patients' homes and communities rather than being tied to a traditional office or hospital setting. Responsibilities:

Looking for someone with a Hospice heart and passion for providing end of life care Provide Case Management to patients and families, this includes symptom management, medication management, and collaboration with members of the IDG Responsible for overseeing patient plan of care with the collaborating physician Provide supervisory visits and oversight for LPN and CNAs Provide education to patients, families, and facility staff Provide oversight to Aides #IND123

Qualifications:

Must have a RN license unincumbered in the state you are applying Reliable transportation and valid driver's license, clean MVR Experience in primary care, home health, or hospice is preferred.
