LinkJobs works with large health care systems, small clinics and everything in between, and we are currently searching for the right person for the following position: Home Visiting Nurse Practitioner (1099 Contractor)Responsibilities:
Manage the transitional and chronic care needs of homebound patients including:- Assessment and evaluation of current condition
- Obtain past medical history
- Review symptoms
- Conduct general physical examinations
- Review and reconcile medications
- Screen for depression
- Screen for alcohol and drug abuse
- Identify diagnoses to support care management and treatment planning
- Recommend appropriate measures, including medication prescriptions and skilled care needs
- Efficiently communicate assessment findings to the office/supervising physician
- Educate patients on disease processes, prevention, medications, and compliance
Qualifications:
- Current Nurse Practitioner license in the state of Georgia with no sanctions or restrictions
- Minimum of 1 year of clinical experience
- Clinical background in Adult, Family, or Geriatric specialties preferred
- Experience using Electronic Health Records (EHR)
- Strong communication skills
- Comfortable using smartphone and email
- Reliable transportation required
- Ability to work independently
- Home care or home visiting experience preferred
- Must maintain active individual malpractice insurance coverage
- Willingness to spend 30 minutes to 1.
